Что означает «hepatic encephalopathy» и как медсёстры её оценивают?

Печёночная энцефалопатия — нейропсихиатрическое осложнение печёночной недостаточности, при котором мозг подвергается воздействию токсинов (главным образом аммиака), которые повреждённая печень не может удалить из портальной крови. Степень варьируется от едва заметных когнитивных изменений (степень 1: нарушение сна, лёгкая спутанность) до глубокой комы (степень 4). Медсёстры оценивают по классификации Уэст-Хейвен, наблюдая ориентацию во времени и пространстве, астериксис (хлопающий тремор кистей при вытянутых руках), чёткость речи и способность выполнять команды.

Как правильно документировать выделения из стомы?

Точная документация стомы должна включать: тип стомы (колостома или илеостома), цвет и консистенцию выделений (для илеостомии используйте Бристольскую шкалу форм стула как ориентир), объём в миллилитрах за каждую смену, состояние кожи вокруг стомы. Для колостомы отмечайте, оформленный ли стул, мягкий или жидкий. При дебите илеостомы более 1500 мл за 24 часа необходимо эскалировать как «высокодебитную стому». Обязательно документируйте целостность мешка, наличие подтеканий и переносимость пациентом процедур самообслуживания.

В чём разница между «melaena» и «haematochezia»?

Оба термина описывают кровотечение из прямой кишки, но из разных источников. Мелена — чёрный, дёгтеобразный, зловонный стул, вызванный переварённой кровью из верхних отделов ЖКТ (проксимальнее илеоцекального клапана): язва желудка, варикоз пищевода. Кровь химически изменяется под действием желудочной кислоты и кишечных бактерий. Гематохезия (ярко-красное ректальное кровотечение) означает выход свежей красной или бордовой крови, как правило, из нижних отделов ЖКТ: геморрой, дивертикулярное кровотечение, колоректальный рак. Это различие определяет маршрут эндоскопии и тактику реанимационных мероприятий.
