В чём разница между «vasopressor» и «inotrope» в практике ОИТ?

Как объяснить «lung-protective ventilation» семье пациента простыми словами?

Можно сказать: «Лёгкие вашего родственника сильно воспалены и хрупки. Аппарат ИВЛ настроен на небольшие мягкие вдохи, чтобы не растягивать ткань лёгких. Между вдохами мы поддерживаем небольшое постоянное давление, чтобы лёгкие оставались открытыми». Что такое определение сепсиса Sepsis-3 и как оно применяется у постели пациента?

Sepsis-3 (2016) определяет сепсис как жизнеугрожающую органную дисфункцию вследствие нарушенного ответа хозяина на инфекцию, где дисфункция органов характеризуется ростом SOFA-счёта на ≥2 балла. Септический шок — подгруппа, требующая вазопрессоров при МАД ≥65 мм рт.ст. и лактате >2 ммоль/л. Инструмент qSOFA используется для быстрого скрининга вне ОИТ. Эти определения заменили критерии SIRS, отражая, что сепсис — прежде всего органная дисфункция.

Когда «continuous renal replacement therapy» предпочтительнее прерывистого гемодиализа?

ПНЗПТ предпочтительна у гемодинамически нестабильных пациентов, не переносящих быстрых сдвигов жидкости и растворённых веществ при прерывистом гемодиализе — например при септическом или кардиогенном шоке. ПНЗПТ медленно и непрерывно устраняет растворённые вещества за 24 часа с меньшей нагрузкой на сердечно-сосудистую систему и позволяет точно управлять балансом жидкости. Решение принимает интенсивист с учётом гемодинамического статуса пациента и возможностей отделения.

Как сообщить о решении «goals of care» ночной смене?

Задокументируйте решение в медицинской карте и плане лечения. При передаче смены чётко изложите имя пациента, диагноз, статус и принятые цели: например, «Пациентка Н., 78 лет, полиорганная недостаточность, день 12. После семейной встречи согласовано ограничение лечения: без эскалации вазопрессоров, без СЛР, фокус на комфорте». Убедитесь, что приказ DNAR чётко подписан, датирован и виден в карте. Избегайте расплывчатых формулировок — используйте официальные задокументированные термины и устно подтвердите, что весь персонал понимает план.
